Get your documents ready

Step 2 to booking

For all appointments, please bring:

  • a copy of the medical form

  • Glasses or contact lenses if worn

  • Details of any medical conditions you have

  • Details of any medications you take

We will advise if there are any other requirements

Step 3 to booking

Attend your appointment

What to bring to your appointment

HGV/LGV/PCV (D4/Lorry/Truck)

  • Photo ID (e.g. passport, driving license)

  • D4 form

  • Driving glasses or contact lenses if worn

  • Details of existing medical conditions (e.g. medical documents)

  • Details of medication currently taken

  • List of all outpatient clinic letters

  • Summary of medical conditions which you can get from your GP

Taxi/Private hire

  • Photo ID (e.g. passport, driving license)

  • An electronic or paper copy of your bank statement or utility bill

  • Taxi medical form from your council

  • Driving glasses or contact lenses if worn

  • Details of existing medical conditions (e.g. medical documents)

  • Details of medication currently taken

  • List of all outpatient clinic letters

  • Summary of medical conditions which you can get from your GP
